The Uptown Health Link collaborative aims to maximize HIV service utilization in Upper Manhattan, increase community awareness and solidarity for effective HIV prevention, promote knowledge of HIV serostatus, and link individuals to targeted HIV prevention, treatment and support services.

Uptown Health Link has identified the disparities to healthcare in the community and has called for mobilization through community partnerships. Selected to collaborate with Harlem United because they are rooted in their neighborhoods with stake in the health outcomes of Upper Manhattan residents our partnering agencies are:

Washington Heights CORNER Project

Dominican Women’s Development Center

National Black Leadership Commission on AIDS

With strong partnership from community agencies, Uptown Health Link will target services in Upper Manhattan as locations being disproportionately affected with HIV and having increased incidence and prevalence of HIV/STI infection compared to other zip codes of Manhattan. Recognizing that the Upper Manhattan Community is experiencing a myriad of public health issues that fuel the rise in infection, including; homelessness, substance use, mental illness, poverty, HIV stigma and homophobia, the project aims to target populations most vulnerable to these conditions.

Uptown Health Link will target MSM of color, young people, women, heterosexuals who engage in HIV risk behaviors but do not perceive individual risk, users of injection and non-injection substances, the homeless and unstably housed.
